"Avoid direct eye contact" is literally the entire counter
Per Destructoid, Stalker's counter is documented in one short sentence: avoid direct eye contact, sanity drops if seen. There's no item that defeats it, no escape mechanic if you've already locked eyes — the damage hits the instant the visual contact happens.
This makes the discipline simple and absolute. Round corners with the camera angled down. Don't try to confirm the sighting. The sighting itself is the failure state.
Where it spawns
Stalker is documented as a corner-and-corridor threat — tall dark figure that emerges from the angle of corridors, particularly the transitional spaces between rooms. It doesn't pursue actively; it stations itself at choke points and waits for the player to round a corner with the camera up.
By Shift 7, Stalker is in regular rotation (per the animalhospital.wiki shifts guide — that's the "veteran nights" band where the enemy becomes a recurring threat).
Why this is the single hardest-to-undo enemy
Other enemies have second chances:
- Skinwalker: Gun / Taser / Fire Extinguisher counters.
- Bed Monster: 30-second Maple Syrup window after grab.
- Mass of Eyes: drop the camera, drain stops.
Stalker has no recovery option. The instant of eye contact = the damage. Which means the only viable defence is preventative camera discipline. By Shift 7, that discipline isn't optional.
What this implies for class choice
The Psychologist class — which doubles sanity changes both ways — is particularly punished by Stalker. Every accidental sighting deals double the standard sanity hit. Pick Psychologist only if you're confident in your camera discipline, not as a learning class for Shift 7+.
